摘要： 目的探索高压氧（HBO）治疗联合尼莫司汀（ACNU）作用对人脑胶质瘤干细胞模型的影响，并初步分析其相关机制。方法人脑胶质瘤干细胞系SU3接种于Balb/c裸小鼠皮下，待肿瘤生长至40 mm3后，随机分为ACNU组、HBO组、ACNU＋HBO组及空白对照组，每组5只。实验期间定期测量荷瘤鼠体质量及肿瘤体积，微电极测量瘤内氧含量变化，治疗结束后取皮下肿瘤称重，观察移植瘤组织形态，并分析乏氧、坏死及血管生成等相关分子表达情况。结果HBO治疗后荷瘤鼠肿瘤组织内氧含量明显升高（P〈0.01），瘤内坏死出血较对照组明显减少。ACNU＋HBO组肿瘤质量最小（1.43±0.38）g，与其他3组差异存在统计学意义（P〈0.05），ACNU＋HBO、ACNU、HBO组的抑瘤率分别为76.29%、43.79%和31.51%。肿瘤坏死因子-α（TNF-α）、缺氧诱导因子-1α（HIF-1α）、血管内皮生长因子（VEGF）和核因子-κB（NF-κB）表达水平和炎细胞浸润光密度值与肿瘤抑制率呈负相关。结论HBO处理可改变肿瘤干细胞赖以生存的乏氧微环境，在一定程度上抑制胶质瘤干细胞启动的肿瘤增殖和肿瘤相关炎细胞浸润，与ACNU联合治疗可提高后者疗效，相关机制与HBO处理逆转肿瘤干细胞巢乏氧、并调控HIF-1α、TNF-α、VEGF和NF-κB表达下降相关。

摘要： 目的 :研究小梁切除术前应用不同眼药水对青光眼患者手术前后炎症状态的影响。方法 :160例行小梁切除术青光眼病例按照术前用眼药水差异,分为观察组(术前予以普拉洛芬眼药水联合氯霉素眼药水治疗)和对照组(术前仅予以氯霉素眼药水)各80例80眼,对比2组手术前后眼部症状与体征综合评分及疗效。结果 :术前2组眼部症状与体征综合评分无显著差异(P>0.05);术后综合评分观察组(2.11±0.13)明显低于对照组(2.54±0.19)(P<0.05);有效率观察组(95.00%)明显高于对照组(85.00%)(P<0.05)。结论 :小梁切除术前予以普拉洛芬眼药水能有效控制青光眼患者手术前后炎症状态,提高手术疗效。

摘要： 目的探讨采用谷氨酰胺联合早期肠内营养治疗危重型胰腺炎对患者炎症反应的影响。方法在2014年6月至2016年4月期间本院收治的危重型胰腺炎患者中选取82例，按照治疗方案的不同将其分为观察组和对照组，其中对照组患者仅接受肠外营养，观察组患者则采用早期肠内营养联合谷氨酰胺进行治疗，对两组患者炎症反应、氨基酸代谢情况进行观察分析。结果观察组患者在治疗后MCP-1[（25．3±3．4）ng/ml]、HMGB-1[（6．7±0．9）ng／ml]水平均明显低于对照组[（41．2±6．3）ng/ml、（10．5±1．7）ng/ml]，差异有统计学意义（P〈0．05）；观察组患者在治疗结束后，其血清苏氨酸[（11．2±1．5）μmol/L]、苯丙氨酸[（4．3±0．6）μmol／L]、缬氨酸[（4．9±0．7）μmol／L]及赖氨酸[（6．6±0．8）μmol／L]水平均明显高于对照组[（7．6±0．9）μmol／L、（2．3±0．4）μmo]]L、（3．5±0．4）μmol／L、（2．7±0．3）μmol／L]，差异有统计学意义（P〈0．05）。结论采用早期肠内营养联合谷氨酰胺治疗危重型胰腺炎，可有效促进患者蛋白质合成的改善，有助于缓解机体负氮平衡，并有利于促进患者全身炎症反应的缓解，建议在临床推广应用。

摘要： Objective: To investigate the effects of Chaiqin Chengqi decoction combined with Western medicine treatment on the immune function, clinical efficacy and acute inflammatory reaction of intestinal obstruction (IO) patients. Methods: 486 IO patients were randomly assigned into the Western medicine treatment group (control group. n=243) and Traditional Chinese medicine (TCM) combined with Western medicine treatment group (treatment group. n=243). The control group was treated with conventional western medicine treatment- while the treatment group was treatment with Chaiqin Chengqi decoction on the basis of control group. Then we detected and compared the serum protein (PA), albumin (Alb), interleukin-6 (IL-6). interleukin-8 (IL-8), tumor necrosis factor alpha (TNF-alpha), white blood cell count (WBC). C reactive protein (CRP) changes of patients in two groups before and after treatment. We also compared the proportion of T lymphocyte subsets, plasma diamine oxidase (DAO) and lactulose / mannitol ratio (LM) changes. Results: The clinical efficacy and the improvement of clinical symptoms in the treatment group was significantly better than in the control group (PO.05). PA. Alb of treatment group increased significantly (P<0.05), yet the levels of IL-6, IL-8. TNF-α, WBC and CRP decreased significantly (P<0.05). DAO and LM were significantly decreased in the treatment group (P<0.05). Compared with before treatment, serum CD4+, CD8+, CD4+/CD8+, IgG and IgA of treatment group significantly increased after treatment (P<0.05). Compared with the control group, serum CD4+, CD8+, CD4+/ CD8+ and IgA of the treatment group increased significantly, and the difference was statistically significant (P<0.05). Conclusion: Chaiqin Chengqi decoction combined with Western medicine can significantly reduce inflammation, improve immune function, and repair the gastrointestinal mucosa in the treatment of patients with intestinal obstruction. 摘要： 目的:分析两性霉素B、内镜手术联合治疗对真菌性鼻-鼻窦炎患者临床疗效、炎症反应、免疫功能的影响。方法:选择2016年5月—2017年5月于我院就诊的真菌性鼻-鼻窦炎患者,共100例,按进入医院的编号分为观察组和对照组,每组50例。对照组进行内镜手术治疗,观察组进行两性霉素B、内镜手术联合治疗,对两组患者的临床疗效、炎症反应、免疫功能等进行科学的比较。结果:观察组各项炎症因子水平明显比对照组低,差异有统计学意义(P<0.05)。观察组的IgG明显比对照组高,差异有统计学意义(P<0.05)。结论:进行在治疗真菌性鼻-鼻窦炎的过程中,采用两性霉素B、内镜手术联合治疗在一定程度上有助于取得较为显著的治疗效果,并且能够有效的降低患者的炎症反应,增强免疫力,值得临床应用与推广。

摘要： 术后慢性疼痛具有发病率高、持续时间长、发病机制复杂等特点,严重影响病人的生活质量。手术所致神经病理性损伤是术后慢性疼痛发生的重要机制。与此同时,手术损伤所致神经系统的过度炎症反应和免疫调节机制亦加快术后慢性疼痛的形成。在术后慢性疼痛的发生发展过程中,神经损伤、坏死组织炎症反应和/或感染均可导致外周神经、DRG及脊髓背角中多种免疫细胞及胶质细胞活化,产生多种促炎症细胞因子和趋化因子,导致组织微环境中促炎症细胞因子和抗炎症细胞因子的比例失衡,外周和中枢神经元兴奋阈值下降,神经元兴奋性提高,发生痛觉敏化。本文主要综述术后神经系统炎症反应与免疫调节所致术后慢性疼痛的相关机制。

摘要： 在机体中,天然免疫应答和炎症反应均受到精准的调控,最终使免疫系统保持稳态。组蛋白修饰是一种表观遗传修饰,它通过调控基因的表达在众多生理过程中起着重要的作用。组蛋白修饰异常与多种疾病如类风湿性关节炎、多发性硬化症和肿瘤等的发病密切相关,并逐渐成为疾病诊断治疗研究的新靶点,然而关于组蛋白修饰在天然免疫应答中发挥的作用目前不是非常清楚。因此,需要进一步探索组蛋白修饰与天然免疫应答及炎症反应之间的关系。

摘要： 目的探讨快速康复外科理念对腹腔镜胃癌根治术患者术后炎症反应及免疫功能的影响。方法回顾性分析2015年5月—2016年5月该院收治的116例纳入标准的腹腔镜胃癌根治术患者资料。其中行快速康复路径的患者60例(观察组),行常规治疗56例(对照组)。比较和分析两组患者术前及术后第1、4、7天C反应蛋白(C reactive protein,CRP)、降钙素原(PCT)、CD4+/CD8+。结果两组患者CRP及PCT均较术前有所增高,对照组增高更明显(P<0.05)。术后第1天观察组CRP:(17.24±10.16)mg/L,对照组CRP:(25.31±18.20)mg/L;术后第4天观察组CRP:(13.72±4.07)mg/L,对照组:(19.82±7.78)mg/L。术后第1天观察组PCT:(1.57±1.31)ng/m L,对照组PCT:(2.27±1.19)ng/m L;术后第4天观察组PCT:(1.43±1.13)ng/m L,对照组PCT:(1.85±0.96)ng/m L。CD4+/CD8+均较术前有所下降,对照组下降更明显(P<0.05)。术后第4天观察组CD4+/CD8+:(1.20±0.22),对照组CD4+/CD8+:(1.10±0.16);术后第7天观察组CD4+/CD8+:(1.24±0.19),对照组CD4+/CD8+:(1.13±0.17)。结论快速康复外科理念有助于促进腹腔镜胃癌根治术患者术后的康复,减轻炎症反应,改善免疫功能。

摘要： To study the effect of CO2 pneumoperitoneum on postoperative inflammatory reaction and immune response of elderly patients with acute appendicitis, and to provide basis for clinical treatment. Methods: 130 elderly patients with acute appendicitis who were treated in our hospital from January 2015 to March 2016 were selected, according to the random number table method,they were divided into study group and control group,with 65 patients in each group. The patients in control group were treated with conventional open appendectomy, while the patients in study group were treated with CO2 pneumoperitoneum laparoscopic appendectomy. The white blood cell count, neutrophil count, interleukin -10 (IL-10) and tumor necrosis factor - α(TNF-α) level were compared between the two groups before and after operation 1 d, 3 d and 5 d. The white blood cell count, neutrophil count, levels of IL-10 and TNF-α in the two groups were significantly increased at 1 d after operation, the differences were statistically significant(P<0.05). Results: The white blood cell count and neutrophil count were compared with the control group at 1 d after operation, the differences were not statistically significant (P>0.05). The levels of TNF-α and IL-10 in the study group were significantly lower than that in the control group at 1 d after operation, the differences were statistically significant (P<0.05); 3 d, 5 d after operation compared with 1 d, the white blood cell count, neutrophil count, levels of IL-10 and TNF-α levels in the two groups were significantly reduced, there was no significant difference between the 3 d after operation and before operation (P>0.05), the difference between the 5 d after operation and before operation was statistically significant(P<0.05). 摘要： Objective To investigate the Relationship between the blood neutrophil-to- lymphocyte ratio and the inflani mation in chronic kidney disease. Methods 116 CKD patients(at stage 3-4)before dialysis(pre-HD group) and 108 GKD patients on maintenance hemodialysis(MHD group) with stable clinical conditions treated in Hainan Hospital Affiliated to Nantong University were recruited for this study. Fifty-four healthy individuals were used as the controls. We measured the whole blood cell count, leukocyte classification and the blood neutmphil-to- lymphocyte ratio(NLR). The blood hs-CRP and Al-B were measured as well as other laboratoiy parameters. rrhe relationship between NLR and other parameters was assessed using Spearman correlation. Results rrhe NLJ{ as well as hs-CRF were significantly higher in MHI) group and pre-HD group as compaid lo the control group, and the two parameters were higher in MHI) group than the pre-HD group(P<0.01 r.Q5). Compared to the control group, the ALB was lower in MHD group and pre-HD group, while the ALB was lower in MHD group compared to the pre-HD group (P<0.01 or P<0.05). In MHD patients v correlation analysis showed the coiTeladon coefficients of NUi to hs-CRP and ALB were r=0.324, P<0.01 ,r=-0.296,p<0.01 ,respectively.
