摘要： A 23-year-old male presented to the emergency room (ER) for polytrauma after involvement in a motor vehicle accident with head trauma but no loss of consciousness. Postsurgery, a neurological examination was also conducted that investigated patient's mental status, cranial nerves, motor function, muscle strength and tone, sensation, reflexes, and coordination which were all negative. The correlation between imaging features and clinical outcome has been analyzed showing a correlation between the size of the cleft and motor deficits and mental retardation, but not seizures.

摘要： Objective: To determine the extent to which the content of the Quality of Life in Neurological Disorders (Neuro-QoL) covers the International Classification of Functioning, Disability and Health (ICF) Core Sets for multiple sclerosis (MS), stroke, spinal cord injury (SCI), and traumatic brain injury (TBI) using summary linkage indicators. Design: Content analysis by linking content of the Neuro-QoL to corresponding ICF codes of each Core Set for MS, stroke, SCI, and TBI. Setting: Three academic centers. Participants: None. Interventions: None. Main Outcome Measures: Four summary linkage indicators proposed by MacDermid et al were estimated to compare the content coverage between Neuro-QoL and the ICF codes of Core Sets for MS, stroke, MS, and TBI. Results: Neuro-QoL represented 20% to 30% Core Set codes for different conditions in which more codes in Core Sets for MS (29%), stroke (28%), and TBI (28%) were covered than those for SCI in the long-term (20%) and early postacute (19%) contexts. Neuro-QoL represented nearly half of the unique Activity and Participation codes (43%-49%) and less than one third of the unique Body Function codes (12%-32%). It represented fewer Environmental Factors codes (2%-6%) and no Body Structures codes. Absolute linkage indicators found that at least 60% of Neuro-QoL items were linked to Core Set codes (63%-95%), but many items covered the same codes as revealed by unique linkage indicators (7%-13%), suggesting high concept redundancy among items. Conclusions: The Neuro-QoL links more closely to ICF Core Sets for stroke, MS, and TBI than to those for SCI, and primarily covers activity and participation ICF domains. Other instruments are needed to address concepts not measured by the Neuro-QoL when a comprehensive health assessment is needed. (C) 2018 by the American Congress of Rehabilitation Medicine

摘要： Background: Headache on wakening (HoW) or sleep interruption secondary to headache (SIH) has been commonly regarded as a potential sign of raised intracranial pressure and therefore a sign of significant underlying pathology that necessitates further investigation. Current recommendations for neuroimaging in patients with HoW/SIH are neither consistent nor clear across headache guidelines published both nationally and internationally. Aim: The main aim of this study was to ascertain the relevance of HoW and/or SIH as an indication for routine neuroimaging. Methods: This study focused on clinically well patients with normal neurological examinations who had experienced HoW or SIH. Demographic and neuroradiological data were collected prospectively and the headache diagnosis was based on the International Classification of Headache Disorders. Results: 102/1065 patients reported either HoW and/or SIH. There were 57/102 (56%) females, 45/102 males (44%), and 33/102 (32%) of ethnic minority. Their age ranged between 5 and 17 years. 79/102 (77%) patients with HoW, 19/102 (19%) with SIH and 4/102 (4%) with both HoW and SIH. Headache diagnosis included migraine (n = 67;66%), tension type headaches (n = 16;16%), medication overuse headaches (n = 11;11%), and sinusitis (n = 1;1%). Neuroimaging was performed in 101/102 patients;imaging was normal for 97 scanned patients, and showed non-significant abnormality in the remaining 4 patients. Conclusion: HoW or SIH among clinically well and neurologically normal paediatric patients was most likely to be caused by primary headaches, particularly migraine or tension type headaches. This symptom alone among healthy and clinically well children is not an indication for routine neuroimaging and is unlikely to be caused by sinister aetiologies. 摘要： The sense of freedom and independence that being able to drive generates may be taken for granted by many until it is threatened by illness. Drawing on the mobility turn' in social sciences that emphasises the social and emotional significance of the car (Sheller and Urry , ), this article presents secondary analysis of narratives of driving and its significance across four neurological conditions (epilepsy, Parkinson's disease, transient ischaemic attack and motor neurone disease). Taking an interactionist approach we explore how the withdrawal of a driving licence can represent not just a practical and emotional loss of independence, but also loss of enjoyment;of a sense and feeling of normal' adulthood and social participation;and of an identity (in some cases gendered) of strength and power. Conversely the ability to keep driving can maintain an unbroken thread of narrative, for example enabling people with speech difficulties to feel and look normal behind the wheel. Moments of pleasure and normality illuminate the importance of examining the micro-strands of disruption illness can cause.

摘要： The article focuses on a study aims to identify specific aspects important for decision making for children with severe neurological impairment (SNI). Topics discussed include process-based model for managing the inherent uncertainty often encountered with treatment decisions, problems resulting from the severely impaired central nervous system (CNS) and requiring noninvasive positive pressure ventilation (NIPPV) with pediatric intensive care unit (PICU).
