摘要： Inflammatory bowel disease (IBD) is the result of a chronic intestinal inflammatory response which usually occurred in colon and small intestine. Keratins constitute the intermediate filament cytoskeleton in all epithelia. The present study was intended to explore the role of Keratin 1 (KRT1) in the progress of IBD. In normal intestinal tissue, the expression of KRT1 was detected by RT-PCR and Western blot. The levels of KRT1 protein significantly decreased in serum samples of IBD patients as compared with sera of healthy controls. Immunohistochemistry revealed that the expression of KRT1 decreased in various intestinal diseases, especially in Crohn's disease and ulcerative colitis. Furthermore, down-regulated KRT1 was correlated with the severity of IBD. The overexpression of KRT1 maintained epithelial barrier in Caco-2 cells after IL-beta treatment. Furthermore, IL-beta-induced disruption of tight junction became significantly attenuated in KRT1 over-expressing Caco-2 cells as compared with control cells. Thus, KRT1 played an important role of maintaining epithelial barrier and its down-regulation in intestinal tissue was correlated with the progression of IBD. (C) 2017 Elsevier B.V. 摘要： Background: The magnitude of risk of serious infections due to available medical therapies of inflammatory bowel disease (IBD) remains controversial. We conducted a systematic review and network meta-analysis of the existing IBD literature to estimate the risk of serious infection in adult IBD patients associated with available medical therapies. Methods: Studies were identified by a literature search of PubMed, Cochrane Library, Medline, Web of Science, Scopus, EMBASE, and ProQuest Dissertations and Theses. Randomized controlled trials comparing IBD medical therapies with no restrictions on language, country of origin, or publication date were included. A network meta-analysis was used to pool direct between treatment comparisons with indirect trial evidence while preserving randomization. Results: Thirty-nine articles fulfilled the inclusion criteria;one study was excluded from the analysis due to disconnectedness. We found no evidence of increased odds of serious infection in comparisons of the different treatment strategies against each other, including combination therapy with a biologic and immunomodulator compared to biologic monotherapy. Similar results were seen in the comparisons between the newer biologics (e.g. vedolizumab) and the anti-tumor necrosis factor agents. Conclusions: No treatment strategy was found to confer a higher risk of serious infection than another, although wide confidence intervals indicate that a clinically significant difference cannot be excluded. These findings provide a better understanding of the risk of serious infection from IBD pharmacotherapy in the adult population.

摘要： Interleukin 33 (IL-33) that signals through the ST2 receptor has emerged as a critical modulator in several inflammatory disorders, including inflammatory bowel disease (IBD). However, the precise mechanisms by which IL-33 modulates IBD are controversial. The aim of this study was thus to clarify the role of IL-33 in IBD. The plasma levels of IL-33 were significantly decreased, but soluble ST2 levels were increased in patients with IBD compared to healthy individuals. Moreover, IL-33 restored goblet cell numbers and induced macrophage switching from the M1 to the M2 phenotype. These effects were sufficient to ameliorate colitis in dextran sodium sulfate, trinitrobenzene sulfonic acid, and peritoneal cavity cell transfer models. IL-33 facilitated goblet cell restoration via modulating macrophages toward the M2 phenotype. In addition, wound healing was significantly faster in IL-33treated human monocyte-derived macrophages than in control cells, which could be attributed to increased polarisation into M2 macrophages. We found that patients with IBD show decreased serum levels of IL-33 compared with healthy individuals and that IL-33 can attenuate colitis and aid tissue repair in mice. The mechanism by which IL-33 exerts these effects appears to involve the stimulation of differentiation of goblet cells and M2 macrophages.

摘要： 目的 探讨复方中药熏洗颗粒对肛肠疾病患者手术治疗后的临床疗效及对术后并发症、创面愈合的影响。方法 选取2013年6月~2016年2月温州市中西医结合医院肛肠科收治的78例肛肠手术患者进行研究,采用随机数字表分为观察组和对照组,各39例,2组患者术后均采用常规的抗炎及换药治疗,观察组同时采用复方中药熏洗颗粒治疗,连续治疗10 d,观察比较2组患者创面愈合及相关临床病症的变化情况。结果 术后第3、7、14天,观察组患者的VAS评分均低于同期对照组,差异均具有统计学意义（P〈0.05）。术后第1天,观察组和对照组患者的肛周水肿、肛门坠胀、肛门潮湿、渗血症状评分差异均无统计学意义;术后第3、7天,观察组患者的肛周水肿、肛门坠胀、肛门潮湿、渗血症状评分均低于同期对照组,差异均具有统计学意义（P〈0.05）。术后第5、14天,观察组患者的创面面积减少率均高于同期对照组,差异均具有统计学意义（P〈0.05）;观察组创面愈合时间平均为（16.3±2.8）天,对照组患者的创面愈合时间平均为（18.4±3.3）天,差异均具有统计学意义（P〈0.05）。结论 肛肠疾病手术治疗后采用复方中药熏洗颗粒进行治疗有助于缓解患者的临床症状、减轻疼痛、促进创面愈合。

摘要： 目的:探讨复方阿嗪米特联合莫沙必利治疗老年胃肠疾病相关性消化不良的疗效。方法:选取2015年3月—2016年8月湖北省荣军医院收治的老年胃肠疾病相关性消化不良患者116例作为研究对象,以随机数字表法分为观察组和对照组,每组58例。对照组患者给予莫沙必利治疗,观察组患者给予复方阿嗪米特联合莫沙必利治疗。观察两组患者的临床疗效、症状消失时间、住院时间、不良反应发生情况,比较两组患者治疗前后的生活质量评分、症状严重程度积分、发病频率的差异。结果:观察组患者的总有效率明显高于对照组,症状消失时间、住院时间明显短于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组患者的生活质量评分、症状严重程度积分、发病频率明显优于对照组,差异均有统计学意义(P<0.05)。两组患者不良反应发生率的差异无统计学意义(P>0.05)。结论:复方阿嗪米特联合莫沙必利治疗老年胃肠疾病相关性消化不良的疗效显著,可有效促进临床症状消退,改善患者生活质量,安全性较高。

摘要： 目的:探讨肛肠疾病术后中药熏洗治疗的疗效和护理。方法:收治肛肠手术患者68例,随机分为治疗组与对照组,各34例。治疗组术后使用中药熏洗治疗,辨证施护;对照组术后使用高锰酸钾溶液熏洗、坐浴。结果:治疗组总有效率高于对照组(P<0.05)。结论:肛肠疾病患者术后采用中药熏洗疗法,能够有效缩减患者恢复时间,疗效明显。
