摘要： The article presents a study which examines the association between psoriasis and inflammatory bowel diseases (IBD). The study showed that the frequency of plaque-type psoriasis, severe psoriasis, psoriatic arthritis and nail psoriasis seems to be lower in patients with IBD compared to non-IBD patients. The association between IBD and psoriasis was found in 43 patients.

摘要： Endoscopy plays a crucial role in the management of inflammatory bowel disease (IBD) in terms of diagnosis, monitoring of mucosal status, and surveillance of colitis-associated neoplasia. Mucosal healing evaluated by endoscopy has been recognized as the target of treatment in the era of powerful biologics therapy. The optimal modality for identifying dysplasia in IBD has yet to be well defined. Increasing progress has recently been made in endoscopic technologies to more accurately assess mucosal inflammation and more effectively detect dysplasia. Here we review the data of advanced endoscopic imaging techniques such as chromoendoscopy, virtual chromoendoscopy, endocytoscopy, and confocal laser endomicroscopy in the management of IBD.

摘要： Inflammatory bowel disease (IBD) is a risk factor for colorectal cancer (CRC). We evaluated chemotherapy tolerance and oncologic outcomes from the medical records of patients with CRC with and without IBD. In this series, patients with CRC with IBD experienced more treatment alterations than those without IBD. Patients with stage IV CRC with IBD had shorter survival than patients without IBD. Background: Inflammatory bowel disease (IBD), comprising Crohn disease and ulcerative colitis, is a risk factor for colorectal cancer (CRC). Chemotherapy toxicity may exacerbate IBD symptoms and vice versa, but data are limited. We evaluated chemotherapy tolerance and oncologic outcomes in patients with CRC with and without IBD. Patients and Methods: Medical records of patients with CRC with and without IBD treated between 2008 and 2013 were reviewed. Where possible, patients were matched by age, sex, stage, and diagnosis year. Chemotherapy tolerance and survival outcomes were compared between patients with IBD and without IBD. Results: A total of 158 subjects with CRC were included: 80 patients had IBD and 78 matched control patients did not have IBD. Between cases and controls, there were no significant differences in demographic data, stage of CRC, and cancer treatments, with equivalent numbers of patients receiving surgery, radiation, and chemotherapy. Patients with IBD experienced more CRC treatment alterations than those without IBD (74% vs. 44%, P = .03), largely due to a higher frequency of treatment delays among patients with IBD. Differences in stage-specific 5-year overall survival (OS) and recurrence-free survival (RFS) in patients with and without IBD were not significant, except for stage IV patients with IBD who had significantly shorter OS than those without IBD. 摘要： The author discusses causes for depression in the setting of inflammatory bowel disease (IBD) in response to an editorial on the impact of a depressive state on outcomes in IBD. The author states potential implications of a research on the broader field of managing psychiatric disorders patients with IBD, and mentions relationship between psychosocial stressors such as depression, and outcomes in IBD. The author notes that gastroenterologists and need training for psychiatric disorders.

摘要： BACKGROUND & AIMS: Mucosal healing, determined by endoscopic evaluation, is one of the most important prognostic markers for patients with inflammatory bowel diseases. Findings from histologic evaluation, however, could complement findings from endoscopy in assessing mucosal responses to treatment. We analyzed long-term results of children treated with thalidomide to determine the association between clinical response and histology and endoscopy findings. METHODS: We collected data from 2 multicenter trials of 70 children with refractory Crohn's disease (CD) or ulcerative colitis (UC) (2-18 years old;ileocolonic or colonic disease) given thalidomide or placebo (NCT00720538). Clinical remission and clinical response at 8 weeks were defined as a pediatric CD activity index scores 10 points or lower and a decrease of at least 50% from baseline, respectively, for patients with CD;and as a pediatric UC activity index score below 10 and a decrease of at least 20 points from baseline, respectively, for patients with UC. Patients with a clinical response to 8 weeks' treatment with thalidomide underwent endoscopic examination with biopsy collection at study weeks 12 and 52. Severity of inflammation in patients with UC was assessed by Mayo score and in patients with CD by 4-grade system. Biopsies were assessed for signs of active inflammation, erosion or ulceration, and crypt abscesses and assigned a histologic score. RESULTS: Clinical remission was observed in 42 patients (60.0%) and clinical response in 45 patients (64.2%) at Week 8. At Week 52, a total of 38 patients (54.3%) were still in clinical remission or still had a clinical response;29 patients (41.4%) had mucosal healing, defined as complete healing of erosions or ulcerations, and 20 patients (27.7%) had histologic healing, defined as complete absence of markers of inflammation. 摘要： OBJECTIVE To evaluate male fertility in Crohn disease (CD) and ulcerative colitis (UC) by examining semen analysis results and paternity from the SHARE study (Subfertility Health Assisted Reproduction and the Environment), a population-based cohort of semen analysis results from Utah men. METHODS A population-based cohort of men with CD or UC was identified using the Utah Population Database (contains person-level linked demographic, genealogical, and medical record information for 85% of Utahans) from 1996 to 2014, and validated by clinical chart review. This cohort was then cross-linked (n = 55) to the SHARE population dataset of semen analysis results. Men with CD or UC were compared with population-based, age-matched, paired (1: 1) controls (n = 47). Paternity was evaluated though presence and number of linked offspring and inter-birth interval. RESULTS Offspring were identified in 71% of UC patients (mean of 1.8 children) and 61% of CD patients (mean of 1.2 children). Compared with matched controls, there were no differences in number of offspring, mean inter-birth interval, or any of the evaluated semen analysis parameters among either men with CD or UC. CONCLUSION Fertility and semen analysis values among men with UC or CD are not significantly impacted compared with population-based, age-matched controls. (C) 2017 Elsevier Inc.

摘要： Background and aims: Perception of quality of care is important in the management of patients with chronic diseases, particularly inflammatory bowel disease. Aims and methods: This longitudinal study aimed to investigate variations of the Quality of Care through the Patients' Eyes (QUOTE-IBD) questionnaire scores one year after the basal evaluation in the Studio Osservazionale quaLita cUre malatTIe crOniche intestiNali (SOLUTION-1) study. Results: Of the cohort of 992 patients, 936 were evaluable. The QUOTE-IBD score overcame satisfactory levels of more than the 80%, overall and in all subdomains except for the "Continuity of Care" sub-dimension (mean, 8.3;standard deviation, 1.49), scored satisfactory only by 34% of the patients. No significant changes in satisfaction were recorded overall, or considering patients subgroups. Significant differences were found at the end of the follow-up between physicians' and patients' perceptions of quality of care, with the former over-rating their performance in "Continuity of Cares" and under-rating "Costs", "Competence", and "Accessibility" sub-domains of the score (p < 0.05 for all). Conclusion: Perceived quality of care in a large cohort of Italian patients with inflammatory bowel disease remains unchanged after one-year follow-up and was not significantly affected by disease activity or therapeutic interventions.
