摘要： 心身疾病是指心理、社会因素在疾病的发生、发展、诊断、预防、治疗及转归等全过程中起着主导作用的一类躯体疾病。社会活动迅速、深刻的变革,大众面临的精神和心理应激增加,心身疾病的患病率呈上升趋势,不当的健康理念、诊疗行为亦在加重这一趋势,心身性肛肠病亦不例外。此前也有学者讨论过此课题[1-3],但肛肠科医师面临着一些交叉学科领域知识的提升和更新。“生物-心理-社会医学”模式这一先进的医学理念的提出和倡导已近40年。如何真正应用于整个诊疗活动中?

摘要： 目的观察电针联合脐贴敷治疗肛肠疾病术后尿潴留的临床疗效。方法将符合研究标准的患者按随机数字表法分为组和对照组,每组各30例,给予电针联合脐贴敷治疗,对照组给予局部热敷治疗。比较两组治疗后至首次排尿时间、首次排尿量及排尿时间,并观察临床疗效。结果治疗后至首次排尿时间、首次排尿量及排尿时间均优于对照组,差异有统计学意义(P <0. 05);和对照组有效率分别为93. 33%及73. 33%,有效率高于对照组,差异有统计学意义(P <0. 05)。结论电针联合脐贴敷治疗肛肠疾病术后尿潴留的临床疗效显著。

摘要： 李廷荃教授受"致中和"思想影响,将其引申结合到"医院管理""中医理论""临床实践""达观处世"中,处处都体现"中和""中庸"之道,其认为"道法自然"一切都回归本原,"守中""守和"才能不偏不倚,做到恰到好处。李教授擅长调理慢性胃肠疾病,笔者跟其出诊,将其"致中和"思想在临证中的运用进行了整理和提炼,以供借鉴。

摘要： Background Low alanine aminotransaminase (LALT) levels may be seen in patients with inflammatory bowel disease (IBD),but there has been no study about the frequency and its clinical *** aimed to analyze the frequency of LALT,and its clinical significance in children with *** The study included the 89 patients with IBD without hepatobiliary *** was defined as ALT levels < 5 U/*** and clinical findings and outcome of the patients with and without LALT were *** LALT was found 47.1% of the *** initial examination,it was more common in female patients (92.3 vs.41.3%,P < 0.001) and patients with CD (57.7 vs.30.2%,P=0.01).75% of the patients with penetrating Crohn's disease (CD) had LALT (P=0.002).Hemoglobin (10.4±2.1 vs.11.7± 1.9 g/dL,P=0.01),folic acid (5.2±3.3 vs.8.6±5.9 ng/mL,P =0.02) and serum albumin levels (3.6 ± 0.8 vs.4.7 ± 5 g/dL,P =0.002) were significantly low in patients with *** was associted with the disease relapse within 2 weeks in 12 of the 16 patients with LALT whereas it was seen in 16 of the 73 patients without LALT during the follow-up (75 vs.21.9%,P < 0.001).Additionally,steroid dependency was more common in patients with LALT during the follow-up (62.5 vs.12.3%,P<0.001).Conclusions LALT is common in children with IBD especially in CD and associated with low hemoglobin,albumin and folic acid *** may be a marker of relapse and steroid dependency.

摘要： Purpose of Review: The purpose of this review is to discuss current knowledge of brain-gut therapies (BGT) in pediatric functional gastrointestinal disorders (FGID) and inflammatory bowel disease (IBD), including their evidence base, the common psychopathology that they address, and the integration of this knowledge into medical settings. Recent Findings: Cognitive behavioral therapy (CBT), hypnotherapy (HT), mindfulness-based therapy (MBT), and exposure-based therapy (EBT) have the most data supporting their use in children, particularly in FGID, more so than in IBD. This difference is most likely because of the increased role of psychological factors in FGID, though these same factors can be seen comorbidly in IBD. Integrative BGT treatment strategies with the collaboration of clinicians across disciplines may provide the most benefit to patients. Summary: This review details our current understanding of the evidence for BGT in pediatric FGID and IBD and how they may best be used in treatment strategies. © 2019, Springer Science+Business Media, LLC, part of Springer Nature.

摘要： 分析胃肠超声造影检查在胃十二指肠疾病筛查方面的应用价值。方法:以2017年12月至2018年12月接受检查的120名胃十二指肠疾病患者为研究对象,收集胃肠超声资料,开展回顾性分析。结果:根据超声诊断结果,120名胃十二指肠疾病患者中胃癌患者人数3例,胃溃疡30例,慢性胃炎患者69人,十二指肠球炎患者15人,十二指肠溃疡患者为13人。90人的胃肠超声检查结果与胃镜检查结果相符。50名患者的诊断结果与病理检查结果相符。结论:胃肠超声造影检查在胃十二指肠疾病筛查领域的应用效果较为显著,具有较为良好的临床应用前景。

摘要： 目的探讨不同干预方法对肛肠疾病骶管麻醉术后排尿时间的影响。方法对185例肛肠疾病患者采用骶管麻醉术后随机分为A、B、C 3组,即:A组(空白对照组)62例、B组(诱导排尿组)61例,C组(电针白环俞组)62例,记录3组术后排尿时间、尿潴留、导尿人数并进行比较。结果 3组术后排尿时间、尿潴留、导尿人数比较,差异有统计学意义(P <0.05),且C组优于B组,B组优于A组。结论诱导排尿、电针白环俞可缩短肛肠疾病骶管麻醉术后排尿时间,电针白环俞效果最佳。

摘要： 探究超声雾化熏洗仪治疗肛肠疾病效果。方法 选取我院肛肠外科自2017年5月至2018年6月肛肠疾病患者100例开展试验,其中试验组为使用超声雾化熏洗仪治疗肛肠疾病患者共74例,对照组为采用温水坐浴患者共26例。结论 超声雾化熏洗仪治疗肛肠疾病有显著效果,建议使用和推广。
